Required Disclosures Outline

The following is a complete list of disclosures required by the Higher Education Act. Many disclosures are required to be updated annually.

The updated disclosures can be obtained from the office of the Education Director, Melodie Dembs, or at the links provided below. All enrolled students receive this list in the Student Handbook prior to enrollment. The school is required to send an updated notice annually. Most SSMT students complete their program within one year, however, any student still enrolled after one year will receive an updated list from a current Student Handbook.

Disclosures Found on the Financial Aid Page

Terms and conditions of the Title IV, HEA loans students receive, including information published by the Department.
A description of all the Federal, State, local, private and institutional student financial assistance programs (need based and non-need based) available to students who enroll at the institution.
For each program, information must include:

Rights and responsibilities of students receiving financial assistance, to include [34 C.F.R. § 668.42(c) except where indicated]
Book Information – Textbook information the International Standard Book Number (ISBN) and retail price information of required and recommended textbooks and supplemental materials.

Summary of requirements related to return of Title IV funds (grant and loan aid)

Services and facilities available to students with disabilities, including students with intellectual disabilities

Academic program information (current degree programs; instructional, lab, or other physical facilities which relate to the program(s); faculty & instructional personnel; and any plans for improving academic programs, if such a plan exists
Gainful Employment – Disclosures related to programs preparing students for gainful employment, including the massage therapy program at SSMT
A list of all programs that are offered at physical campus locations (there is only one, the 750 clock hour massage therapy program)
